Mental Health Survey

The Mental Health and Spiritual Care Committee of the Brooklyn Long Term Recovery Group is currently compiling a referral list for mental health and social service providers working in Sandy-affected areas of Brooklyn. With this in mind, we are asking mental health, emotional care, and social service providers to fill out this survey about their organizations and services (if you would like to be included on the referral list): https://docs.google.com/forms/d/175LOSburBhox-CHiLA2fCk8cpCU3GEc-aNc-nMf6Ijc/viewform

Once compiled, this referral list will be distributed among Disaster Case Managers and other front line service providers. We will also be putting together a flyer for the public from this information.

If you have questions or comments, please contact Jeff Mathias at jmathias@health.nyc.gov

Hope all is well,

The Mental Health and Spiritual Committee
